NIHR BioResource Questionnaire - dosage list

The NIHR BioResource questionnaire includes a section for participants to list their long-term medication.

Our intention is to build up a picking list for this, compiled from the answers given by participants, and add to it as the recruitment process expands.

There is a dm+d browser at ​http://www.ppa.org.uk/systems/pcddbrowserv2_3new/browser.jsp#product which is a good place to look up correct drug names and dose information.

We should have a text entry for the number and a radio button to select the units between gram/milligram/microgram/milliltre/puff
