HSCIS Data Access
Details are available from the HSCIS Data access request website.
It seems that there are several forms to complete. So many, indeed, that it requires the use of a HSCIC Data Access Request Form Matrix.
There is a charge detailed in the HSCIC Data Access service chrages PDF, which can be quite significant depending on what data you request.
