    41 I've emailed Velina to ask for some clarification on any additional activities needed, and whether patients will be able to submit questionnaires direct to Leeds, or complete the data collection online.
     42Study documentation implies online data entry by patients, but for the foreseeable future this will not be happening.

     44Probably do need additional activity for "MINAP data submitted to LEEDS", at around 120 days post-discharge.

     46Each q'aire also has an activity of 'check completeness', and a further activity for 'copy sent to Leeds'.
